Primary Care Physician forms (aka PCP forms) should be submitted back to LabCorp Employer Services in one of the following ways.
You as the participant should also always receive a copy of the screening results and completed PCP form for your records.
By upload via the WellConnect+ site
All you have to do is click 'Submit Form' and you will be able to place a PDF or other scan.
NOTE: This feature does not work well with high resolution images due to file size limitation. Please reduce the file size or attempt fax.
Traditional Fax
Each PCP form has a client specific fax number that should be used and that fax number can be found on the cover sheet of the PCP file that you downloaded.
Your doctor should review the form carefully, complete all fields (including the proper screening date and year) and sign the form where indicated. Once signed they should fax to the number indicated. If they are having issues, they can try the general fax line at 888.972.1871
NOTE: If you doctor cannot or will not fax the form you can also take a scanned copy and use a free online service like Fax Zero to complete the form submission.
